FOR PROFESSIONALS
A collaborative partner in your client’s care
Supporting an adolescent or young adult with complex needs often requires several perspectives working toward a shared goal. I partner with therapists, psychiatrists, medical providers, educators, and other professionals to bring those perspectives together and help translate treatment and recommendations to support actionable steps. For teams that are uncertain about next steps, I work with teams to make recommendations and create a plan.
You may be seeking additional support when a client’s progress has stalled, when their needs extend beyond the resources currently in place, or when a placement or transition requires careful planning. I begin by listening to the professionals involved, understanding the work already underway, and exploring where I can contribute.
When to connect on a case
-
A client’s progress has stalled or their needs have changed.
-
The family needs help understanding and acting on recommendations.
-
Additional providers or resources may be needed.
-
A psychiatric, therapeutic, or educational placement is being considered.
-
A client is preparing for school re-entry, college, or another transition.
-
A client is launching into young adulthood and would benefit from targeted transitional support.
How I support your work
-
Developing a fuller picture: Gathering insights from the client, family, and professionals to understand strengths, needs, and barriers to progress.
-
Coordinating care: Facilitating communication, clarifying priorities, and helping the team work toward a shared plan.
-
Building the team: Identifying and vetting additional providers whose expertise and collaborative approach fit the client’s needs.
-
Guiding placements and transitions: Coordinating with programs, schools, and outpatient providers to support continuity of care.
-
Supporting follow-through: Helping clients and families put recommendations into practice across everyday settings.
Throughout our work together, I bring respect for each professional’s expertise and a commitment to keeping the client’s goals and voice central to the process, while considering historical and familial context, clinical acuity, and the demands of the environments they are transitioning into.
